What to bring with you to a drop-in clinic
When attending a clinic, you must bring along relevant documents to help the occupational therapist assess you, such as:
- letters which detail any diagnoses or health conditions
- prescriptions and lists of medications
- photos of relevant parts of the property or measurements of furniture you wish to discuss
It would also be helpful if you brought details of the following things, if relevant
- details of your GP Surgery
- any package of care from social care you have, including any professional carers and/or family support
- existing equipment in place in your home, and/or mobility aides being used (including walking sticks or wheelchairs)
- your current height and weight
These details will help the Occupational Therapist identify what is the right support for you at the clinic.
